Update LDAP-related module documentation to use DOMAIN/USERNAME/PASSWORD instead of BIND_DN/BIND_PW options

These module documentations were not fully updated when we switched BIND_DN and BIND_PW options to DOMAIN, USERNAME and PASSWORD: #16700

The references to BIND_DN and BIND_PW should be replaced by DOMAIN``, USERNAMEandPASSWORD. An alternative is to include the domain in USERNAME: @`.
