Move non-subgraph ids to CAIP standards #744

we should consider moving all non-subgraph plugin id generation to CAIP standards like i prototyped in the ensv2 refactor branch here
CAIP is a standard for addressing accounts, contracts, nfts, etc and is chain-scoped by default. it would replace our custom concatenation behavior for some id generation.
but that said it's not really a big deal, just a nice standard to adopt if we wanted to
