Document publish command in README

It is not true that Issue can only _pull_ and cannot push data.

README should acknowledge the fact that there are `fetch` (for fetching data from remotes) and  `publish` (for, well, publishing data to remotes).
Also, it should describe types of nodes in example Issue network (i.e. _endpoints_ and _exchanges_) and explain the difference between them.
