[Enhancement] Accept TOS / Disclaimer form banner before participation in custom contest

Currently testers are obligated to fill a Google Forms Form to accept TOS. This is **not** a good practice, since personal sensitive information is transferred trough insecure channel.

It is a good idea to include custom TOS / Disclaimer form banner/window whatever before participation in a contest. This should be *optional* and will be useful in other cases too.
