Tracking upstream bugs

This issue is just a place to link upstream bugs in Swift that might affect the output of this tool. Here are the ones I currently know about:

• Index data usr wrong when extending a typealias swiftlang/swift#64594
• [SR-12096] Index missing references to initializers in Result extensions swiftlang/swift#54532
• [SR-13857] indexstore missing references for superclass / protocol when using composition swiftlang/swift#56255
• [SR-13954] Sourcekit index request doesn't include polluted transitive dependencies swiftlang/swift#56351
• [SR-13959] Protocol conformance in other module not present in sourcekit index swiftlang/swift#56356

(I haven't recently verified the older issues there, so it's possible those are fixed, if so it would be nice to verify and close them upstream)
